CalSTRS. Share these steps with your Spouse / RDP to complete this process.
Your Spouse / RDP must:
Log in to
my
CalSTRS.
For first time users:
Select the
Register Now
button from the
my.calstrs.com
Log In
screen.
For
my
CalSTRS users (with login):
Log in and select
View Application Status & Instructions
from the
myCalSTRS Online Services
screen.
View the list of forms and applications that are pending spousal electronic signature (e-signature).
Complete the steps required to provide e-signature and select
Submit
to send to CalSTRS.

Submit a Completed Express Benefit Report Form to Your Last Employer
The
Express Benefit Report (SR 0554E)
form is required for retirement. Now that you have submitted your
Service Retirement
application, you must complete
Section 1: Member Information
of the
Express Benefit Report (SR 0554E)
form and submit it to your last employer.
This form requests accumulated unused sick leave balance information from your last employer.
Your last employer must report and certify required employment information on this form to comply with California law.
Credit for unused sick leave is included in your final retirement benefit calculation.
